Why Continuous Third-Party Monitoring Became Non-Negotiable

We started noticing something uncomfortable in our research about two years ago.

Browser extensions and OAuth apps that passed initial security reviews were introducing new vulnerabilities within weeks. Apps that looked safe at approval time were escalating permissions through silent updates. Organizations running quarterly security audits were discovering compromises months after the damage window had closed.

The gap between “when we checked” and “when it changed” kept widening.

Across the 400,000+ apps and browser extensions we’ve analyzed, the pattern is consistent: the “safe” window after initial vetting is measured in weeks to a few months, not years. In real compromise cases like the Cyberhaven campaign, attackers pushed malicious updates that stayed live for days to roughly three months before full takedown.

That timeline sits squarely inside the standard quarterly audit cycle.

Organizations operating on 90-day security reviews are systematically discovering threats after the operational window has passed. The control and the threat model no longer match.

The Authorization-to-Risk Timeline Collapsed

Initial vetting is a point-in-time filter.

An extension passes review. It gets approved. It sits in your environment doing exactly what it’s supposed to do. Then it updates.

A meaningful subset of extensions shift from low to high risk via updates within one cycle (typically weeks to a few months) and can remain in that risky state for one to three months before detection and removal.

When we brief security teams on this timeline compression, the reaction follows a pattern. They’re not shocked that extensions can go bad between audits. Most practitioners already know store vetting isn’t enough.

What surprises them is the measured dwell time.

Malicious updates staying live for roughly 90 days on average from compromise to patch or removal in real campaigns. That number lands hard because it almost perfectly matches their audit cadence.

They’ve been treating a quarterly control as if it were continuous.

The other data point that tends to shift the conversation: nearly 50% of browser extensions in enterprise environments are classified as high-risk, with tens of thousands installed from unknown or personal-email developers.

Seeing hard data that malicious or over-privileged behavior can sit in that 30-to-90-day window between audits (and that more than half of installed extensions carry medium or high risk) turns a nagging concern into a structural problem.

Shadow Updates Are the Real Problem

We used to talk about shadow IT as the primary risk.

Employees installing unapproved tools. Departments spinning up their own SaaS subscriptions. The security team discovering entire application ecosystems they never knew existed.

Yes, those are still big problems, but they’re not what’s aligned with how threats actually move.

The hidden issue, which is the biggest threat to even the most mature security programs, is shadow updates: extensions and OAuth apps that pass initial vetting, get approved, and then introduce vulnerabilities within days through automatic updates that security teams never see coming.

You’re not monitoring installation anymore. You’re monitoring evolution.

In organizations that moved from quarterly audits to continuous monitoring, the effective dwell time for risky or compromised extensions collapses from roughly 90 days down to hours or days for most meaningful events.

When an extension’s code or permissions change, or its reputation drops (ownership change, new CVEs, breach news) that shows up in minutes to hours, not at the next 90-day review.

In the malicious-update campaigns we’ve analyzed, continuous monitoring would have identified suspicious behavior and version changes weeks earlier, turning a 98-day average exposure window into something on the order of a few days between bad update and detection or removal.

That shift (from “we find out quarterly” to “we see behavior change as it happens”) is why browser extensions need to be treated like any other active threat surface.

You need continuous telemetry and policy, not a 90-day clipboard, because the real attack window sits squarely inside that quarter.

What Actually Triggers an Alert

When an extension updates and changes permissions or behavior, we’re tracking a combination of permission changes, behavioral anomalies, and reputation shifts that together say “this extension is no longer the one you approved.”

In the first few hours after an update, alerts are typically driven by three specific signal categories.

Permission and Capability Changes

When a new version lands, we diff what the extension can do against what you previously approved.

New high-risk permissions trigger alerts: gaining access to all URLs or all tabs, adding webRequest or proxy capabilities that let it intercept or modify traffic, adding identity, cookie, or storage APIs tied to SaaS apps like Gmail, M365, or CRM systems.

Scope creep versus stated purpose also matters.

A grammar checker suddenly asking for full read/write on every page and email. A wallpaper or emoji extension starting to request identity and network interception.

Those permission diffs get evaluated against policy. If the new set jumps risk tiers or violates least-privilege rules, we raise the risk score and can auto-block or quarantine by policy.

Behavioral and Installation Anomalies

We also watch what the updated extension actually does in the browser and network.

Unusual data access patterns show up as alerts: sudden spikes in reading document object model (DOM) content, cookies, or session data across SaaS domains. New or increased calls to external domains not seen in prior versions.

Suspicious network behavior triggers flags: use of webRequest or proxy to intercept traffic for login pages or APIs. Large or frequent data exfiltration patterns to previously unseen endpoints.

Fleet-wide install and update patterns matter too. Rapid propagation across users or devices outside expected channels—sideloaded CRX files or installs outside your allowlist. Extensions removed from the Chrome or Edge store but still present in your fleet.

These show up as anomaly alerts: “Extension X began accessing Y SaaS domains or sending data to Z host after version N.m.”

That’s often the earliest sign something changed beyond a benign feature update.

Reputation and Ownership Signals

We incorporate threat intel and ecosystem metadata.

Publisher and ownership changes: developer account changes hands, moves to a throwaway domain, or shows signs of compromise. Store and community signals: extension pulled from the store, new bad reviews mentioning fraud, or flags from other security feeds.

Known campaign indicators: hashes, IDs, or domains linked to ongoing malicious extension campaigns—look-alike ChatGPT tools, fake VPNs—matched against your installed base.

When those signals cross a policy threshold—new high-risk permission plus new exfiltration domain, or publisher change plus store removal—that triggers early alerts and, in many environments, automatic actions.

Disabling the extension. Blocking further installs. Kicking off a focused investigation on affected users and data.

The False-Positive Problem

Most raw permission changes are benign on their own.

What keeps the false-positive rate manageable is that we don’t alert on “permission diff” alone. We alert when that change is coupled with risk context.

Many extensions legitimately add permissions—new features, Manifest V2 to V3 migration, broader site support. A simple “any new permission equals incident” rule would be noisy.

The risk model instead re-scores the extension across 20+ factors: business purpose, publisher, external calls, update history, compliance posture. It tracks score deltas over time.

Alerts fire on “permission change plus risk jump.”

New high-risk permission and new outbound domains. Permission expansion misaligned with the extension’s stated business function. Permission change coinciding with store removal, publisher change, or known indicators of compromise.

In real deployments, that multi-signal approach means most routine permission escalations from well-known vendors show up as score changes without blocking alerts.

Admins can review them in the console or reports, but they don’t trigger incident noise unless other risk factors move with them.

The small subset that do trigger automated policies are the ones where the extension’s risk score crosses a threshold—for example from low or medium to high—which almost always correlates with objectively risky behavior like data exfiltration, suspicious domains, or ownership anomalies rather than just a new API in the manifest.

Security teams mostly see permission changes as context for risk, not as standalone alarms, unless they come bundled with other signals that justify treating the update as potentially compromised.

How Organizations Actually Remediate

Most teams want to hit “nuke all high-risk extensions” when they first turn on continuous monitoring and see hundreds or thousands of them.

The organizations that succeed take a phased, policy-driven approach instead of trying to clean house in one shot.

The remediation path typically follows four stages.

Immediate Block on Obvious Outliers

First wave is low-regret removals.

Kill extensions with known malicious indicators of compromise, store removals, or clearly abusive categories (coupon scrapers, video downloaders on clinical devices, shady GenAI data-slurpers).

Auto-remove or deny anything with unknown or no-name Gmail authors plus high-risk permissions, unless it’s on an explicit allowlist.

This can often remove 10 to 20% of the riskiest set in days without breaking workflows.

Policy Line in the Sand

Next, they stop the problem from getting worse.

Enforce allowlists and denylists by category and risk score: only approved password managers, no consumer VPNs, no generic screen recorders on PHI-handling machines.

Require a documented business justification and owner for any high-risk extension that remains, turning “shadow” extensions into accountable ones.

From that point, new risky extensions mostly can’t creep in without someone noticing.

Tiered Cleanup of the Remaining High-Risk Set

For the rest, they work in prioritized waves instead of a big bang.

Slice by risk times blast radius: focus first on high-risk extensions installed on high-value users or devices (clinical, finance, executives) then widen.

Use automation to propose safer alternatives, corporate-approved tools, so security isn’t negotiating one-off replacements by hand with every team.

Each wave removes or replaces another chunk of high-risk extensions while keeping the business running.

Continuous Re-Evaluation Instead of “Done”

Finally, they accept that the surface is dynamic.

Keep continuous risk scoring on for all installed extensions so permission creep, ownership changes, or new exploits push items back onto the remediation list.

Tie this into browser and endpoint policy: “No extension may access corporate SaaS domains unless its risk score is below X or it’s on the allowlist.” Enforcement becomes automatic, not ticket-driven.

The pattern isn’t “clean everything at once.” It’s: stop new risk, remove the worst offenders fast, then iteratively shrink the high-risk footprint with clear policies and automation.

The surprise for most teams is how much they can safely remove in that first and second wave once they see the data laid out by risk and business owner instead of as one undifferentiated wall of red.

Who Owns the Hard Conversation

The smoothest programs put security in the lead, IT in the loop, and compliance as the forcing function.

They’re very explicit about who says what to whom.

In organizations that move fast without chaos, security owns the risk call and the policy: which categories and permissions are unacceptable, which extensions are blocked by default, and why this matters for data and SaaS.

IT or end-user computing owns the change management: communication to finance or clinical teams, deployment via Chrome or Edge policies, and handling the “my extension vanished” tickets.

Compliance or privacy provides the non-negotiable backing: tying extension cleanup to HIPAA, PCI, or AI-policy requirements and third-party risk, so it’s clearly not just security being difficult.

When finance or clinical groups hear “half your extensions are going away,” the message ideally comes from IT with compliance’s authority behind it and security’s data underneath it, not from security alone.

Boundaries matter for how fast you can remediate.

Where ownership is fuzzy (IT lets people install what they want, security disapproves, compliance is quiet) every block turns into a negotiation with each department, and cleanup drags out for quarters.

Where there’s a formal, centralized extension policy (security defines risk thresholds, compliance signs off, IT enforces via Chrome Enterprise, Intune, or SpinCRX) the first two remediation waves can be done in weeks with relatively little back-and-forth.

The key structural move is agreeing up front that security decides what is risky, compliance decides what regulations require, IT decides how and when to roll it out, and all three stand behind a single, written extension policy that business units can’t quietly opt out of.

Once that’s in place, you still have friction. Power users will miss their tools.

But you don’t have policy arguments on every ticket, and the conversation shifts from “why is security doing this to us?” to “here’s the enterprise standard, here’s the safer alternative, here’s the date this class of extensions will no longer be allowed.”

How Compliance Frameworks Encoded Continuous Monitoring

Regulators didn’t suddenly add the words “continuous monitoring” to every framework.

What changed is how auditors and data protection authorities started interpreting existing “ongoing” and “state of the art” language in a world of SaaS and browser-based risk.

The shift happened in three ways.

Risk-Based Clauses Started to Mean “Near Real Time” in SaaS

GDPR always talked about evaluating risks and implementing measures “taking into account the state of the art” and the nature of processing.

As enforcement ramped up (especially around client-side tracking, third-party scripts, and AI) authorities and guidance began to explicitly call out real-time or continuous monitoring of browser-side behavior and third-party code as an expectation for modern SaaS, not a nice-to-have.

SOC 2 and Similar Frameworks Became De Facto Continuous in Practice

SOC 2 already requires controls to operate over the entire audit period, but tooling and auditor expectations have shifted from “show me quarterly samples” to “show me evidence that you had continuous visibility and alerting on SaaS access, third-party apps, and browser risk during the full year.”

Recent SOC 2 guidance describes it as a continuous monitoring framework rather than a once-a-year check.

Front-End and Integration Risk Forced the Issue

As breaches and enforcement started focusing on third-party scripts, OAuth apps, and browser extensions, regulators and customers began asking questions you simply can’t answer with quarterly audits.

“Can you demonstrate ongoing monitoring of client-side data collection?”

“How do you track risky SaaS integrations and extensions over time, not just at onboarding?”

The language that’s “encoding” continuous monitoring is mostly already on the books (risk-based security, appropriate to the state of the art, controls operating over the audit period).

But enforcement, guidance, and customer expectations in 2024 through 2026 have tightened around SaaS and browser surfaces to the point where point-in-time and quarterly looks are no longer seen as sufficient for those domains.

Organizations are being pushed (by auditors, data protection authorities, and large customers) to show ongoing, evidence-backed visibility into SaaS configurations, third-party apps, and browser extensions.

That’s exactly where continuous monitoring has gone from best practice to de facto requirement.

What Tips Organizations Into Adoption

The tipping point is almost never a theoretical argument.

It’s when a very real gap between “quarterly” and “SaaS-speed” becomes painfully visible in one of three ways: a stealthy incident, an audit that asks for evidence they don’t have, or a customer or regulator question they can’t answer with point-in-time data.

A “We Only Noticed It Months Later” Incident

For many, the real trigger is an event that clearly sat in the environment between audit windows.

A malicious browser extension or OAuth app that was benign at approval time, updated silently, and ran for weeks to months before anyone spotted unusual behavior.

A SaaS ransomware or mass-delete pattern that, in hindsight, began days earlier with low-and-slow activity no quarterly control was watching.

In the post-incident review, someone inevitably asks, “When did this actually start, and what did we have in place that day?”

The answer is: “We would only have seen it at the next quarterly review.”

That’s usually the moment quarterly is no longer defensible.

An Audit That Demands “Over the Period” Proof

Another common trigger is an audit where the old way of demonstrating control—screenshots and a few samples—no longer satisfies the auditor.

SOC 2 and ISO reviewers increasingly ask for evidence that SaaS access, third-party apps, and browser risks were monitored continuously over the audit period, not just attested via annual reviews.

When a team can’t produce logs or alerts that show ongoing monitoring (for example, around extension installs and updates or risky SaaS integrations) the finding might be phrased as a gap in operating effectiveness, even if the policy says the right things.

A “soft fail” like that (controls defined but not continuously evidenced) often becomes the internal catalyst for investing in continuous monitoring.

A Regulator or Major Customer Asking SaaS-Specific Questions

The third pattern is pressure from outside.

A data protection authority, regulator, or big enterprise customer asks questions like, “How do you monitor third-party apps and browser extensions that can access our data in real time?” or “Show us how you detect configuration drift in SaaS between annual reviews.”

When the answer is essentially “we rely on quarterly audits and manual reviews,” it becomes obvious that this doesn’t meet modern expectations for “state of the art” in a cloud-first environment.

Once that gap shows up in a contractual negotiation or data protection impact assessment, continuous monitoring moves from “roadmap” to “prerequisite to close business and stay compliant.”

While breaches are a big driver, the more subtle but equally powerful triggers are an incident timeline that clearly slips between audit checkpoints, an audit that demands period-long evidence instead of point-in-time snapshots, or a regulator or key customer explicitly expecting SaaS and browser-level continuous visibility.

That’s when organizations stop treating continuous monitoring as a nice add-on and start treating it as the only credible way to prove their controls actually operate in the world they now live in.

How Audits Change With Continuous Evidence

What changes most is that audits stop being archaeology and start being journalism.

Instead of reconstructing what might have been happening between checkpoints, you’re handing auditors a continuous narrative with timestamps.

Three things we see consistently once continuous monitoring is in place.

Prep Time Compresses Dramatically

With continuous evidence collection from SaaS, extensions, and integrations, teams spend far less time screenshotting consoles and pulling ad-hoc samples.

Evidence for “over the period” controls (like SaaS posture, third-party access, and browser risk) can be exported directly from the monitoring platform.

That turns months of manual audit prep into weeks or even days of report selection and review.

Findings Shift From “Visibility Gaps” to “Tuning Gaps”

Auditors who used to write up “no effective mechanism to monitor X between reviews” now see continuous logs, alerts, and posture checks for SaaS apps and extensions.

The conversation moves away from “you don’t have visibility” toward “are your thresholds and workflows appropriate?”

That’s a much better place to be. It typically reduces both the number and severity of findings around SaaS and browser surfaces.

Auditors Raise the Bar, But You’re Ready for It

Once you can show continuous monitoring, auditors start asking richer questions.

“How quickly do you respond to misconfigurations or risky apps when detected?”

“Can you demonstrate improvement in posture over the audit period?”

Because the telemetry is already there, answering those becomes a matter of running reports, not spinning up special projects.

Audits become more about control performance over time and less about proving the controls exist at all.

So yes, it tends to compress timelines and reduce “we can’t prove this” findings, but the deeper change is qualitative.

Audits become a review of a live, data-backed security program instead of a scavenger hunt for quarterly snapshots that everyone knows don’t reflect what actually happened in the gaps.

Where We Are on the Adoption Curve

We’re in the early-majority phase for continuous monitoring in SaaS and browser risk.

Well past the pioneers, but still with a long tail of laggards treating it as optional.

Most larger or regulated organizations now accept that real-time or near-real-time visibility is table stakes for SaaS posture, third-party apps, and browser extensions. It’s built into modern SSPM and SaaS security tool evaluations.

But there’s still a big segment (especially mid-market and traditionally on-premises-centric sectors) running largely on periodic reviews and hoping their SIEM fills the gaps.

The forcing functions that will push laggards over the line are the same ones we’ve been discussing, just more visible and frequent.

Audit and compliance pressure: SOC 2, ISO 27001, HIPAA, and GDPR programs are increasingly expecting continuous evidence, not annual screenshots, particularly for SaaS and client-side risk. That’s already showing up in checklists and guidance that call out automated evidence collection and continuous control monitoring as best practice.

Customer-driven requirements: Enterprise buyers (especially in healthcare, finance, and EU markets) are adding explicit questions about continuous SaaS and browser monitoring into security questionnaires and contracts. “Do you continuously monitor SaaS misconfigurations and third-party access?” is becoming a standard ask, not a niche one.

Incidents that clearly fall between snapshots: As more breaches involve OAuth apps, SaaS-to-SaaS integrations, AI agents, and browser extensions, it becomes obvious when something lived unnoticed for weeks or months between quarterly checks. Each high-profile case like that makes “point-in-time only” harder to defend to boards and regulators.

The curve right now: leaders and early majority are already operating with continuous monitoring as the baseline. The laggards will likely move not because the frameworks change, but because audits, customers, and incidents converge to make it clear that you simply can’t run a SaaS-first business on snapshot-era assumptions anymore.

Continuous monitoring will look, in hindsight, like MFA: once “advanced,” then suddenly just how things are done.

Start With What You Can Monitor Today

If you’re still operating on quarterly audits, the gap between your control cadence and the actual threat timeline is widening.

You don’t need to rebuild your entire security stack to start closing it.

Begin with continuous risk scoring for browser extensions and OAuth apps. Those are the surfaces where the authorization-to-risk timeline has collapsed most dramatically, and they’re also the ones where most organizations have the least visibility between reviews.

Set up automated alerts for permission changes, behavioral anomalies, and reputation shifts. You’re not trying to catch everything on day one. You’re trying to compress that 90-day dwell time down to days or hours for the events that matter most.

Define a phased remediation path before you turn on monitoring. Agree with IT, security, and compliance on who owns what, which categories are immediate blocks, and how you’ll handle the rest in prioritized waves.

Tie continuous monitoring into your audit prep. The next time an auditor asks for “over the period” evidence, you’ll have logs and alerts to export instead of scrambling to reconstruct what might have happened between checkpoints.

The organizations that moved first aren’t smarter. They just recognized earlier that the old model (vet once, trust indefinitely, check quarterly) no longer matches how SaaS environments actually behave.

Continuous monitoring isn’t a competitive advantage anymore. It’s the baseline for operating in a world where threats update faster than your audit cycles.
